Box 13: Third Party Sick Pay

Do I report this on my 2009 1040?

I was out of work and received third party checks, with the FICA adjustment. There wasn't FITW adjustment. If I have to report this on my 1040, how do I do this? I did not receive any end of the year documentation from the company to help me report this. The amount received less the FICA, was approximately $1600.00.

Check with your employer.  Sometimes third party sick pay ends up being included in your W2 from your employer.

If that was not the case, then you will need to contact the third party to ask for another copy of your W2 from them.
